A plate past the wear limit keeps running because the change-out is planned for next month and the cavity 'looks fine from above'. The round still closes, the number never lands on the asset, and the next person inherits a guess. Do not enter the crusher cavity.
Weekly, locked out. A plate at the site's wear limit is a planned change-out, not a running adjustment. An as-found reading outside the limit the site already published is a finding. It does not become a new setpoint on this work order.
